48 hour GP reporting form

First of all you should know that we'll be using this information in complete confidence. Although we collect your name and as much of your address as you feel comfortable providing, none of this private information will be passed onto your GP, Primary Care Trust or the Government. Our purpose is merely to measure the true extent of the problem and use the data to press the Government to understand the reality on the ground.

This form should be used by anyone who has recently tried and failed to receive a GP appointment within 48 hours.
